Summary

Феномен атопічного маршу описує закономірну та послідовну     еволюцію алергічних станів, що зазвичай бере свій початок у ранньому дитячому віці з проявів атопічного дерматиту. У подальшому цей каскад трансформується у харчову сенсибілізацію, алергічний риніт та, як фінальний і найбільш тяжкий етап, бронхіальну астму. Дана стаття присвячена детальному аналізу патогенетичних механізмів, що лежать в основі цієї прогресії, зокрема критичній ролі дефектів епідермального бар’єру та домінуванню системної Th2-імунної відповіді. Розглядається гіпотеза «шкіри як вхідних воріт» для зовнішніх алергенів, що призводить до первинної сенсибілізації та подальшої системної гіперреактивності слизових оболонок. У роботі детально проаналізовано сучасні методи профілактики, від інтенсивного догляду за епідермісом до застосування новітньої біологічної терапії. Окрему увагу приділено генетичним детермінантам, зокрема мутаціям гена філагрину, та впливу мікробіому на стабільність імунної регуляції. Стаття обґрунтовує необхідність раннього мультидисциплінарного втручання для запобігання хронізації процесу та розширення спектру атопічних захворювань.
