Хронічне обструктивне захворювання легень (ХОЗЛ) традиційно розглядалося як патологія з переважно нейтрофільним запаленням, проте в останнє десятиліття відбувається переосмислення гетерогенності цього захворювання. Приблизно у 20–40% пацієнтів з ХОЗЛ виявляється еозинофільний фенотип запалення, асоційований з Т2-імунною відповіддю. Це відкриття створило підґрунтя для застосування таргетних біологічних препаратів, які вже довели свою ефективність при бронхіальній астмі.
Еозинофільний фенотип хронічної обструктивної хвороби легень (ХОЗЛ) характеризується підвищеним рівнем еозинофілів у периферичній крові або дихальних шляхах, асоційованим із підвищеним ризиком загострень, кращою відповіддю на кортикостероїди та відмінною патогенетичною основою від класичного нейтрофільного запалення. Моноклональні антитіла, що блокують IL-5 (mepolizumab, benralizumab) або IL-4/13 сигналізацію (dupilumab), пропонують можливість таргетної імунотерапії, спрямованої на модулювання специфічних шляхів еозинофільного запалення [1,2].
