Ішемічна хвороба серця (ІХС) залишається однією з провідних причин смертності та інвалідизації населення у всьому світі. Провідну роль у патогенезі атеросклеротичного ураження коронарних артерій відіграє дисліпідемія, зокрема підвищений рівень холестерину ліпопротеїнів низької щільності (ЛПНЩ). Сучасні дослідження доводять, що інтенсивна гіполіпідемічна терапія здатна не лише знижувати концентрацію ЛПНЩ, але й впливати на структуру та стабільність атеросклеротичної бляшки. Досягнення низьких цільових рівнів ЛПНЩ асоціюється зі зменшенням ліпідного ядра бляшки, збільшенням товщини фіброзної покришки та зниженням запальної активності. У клінічних дослідженнях показано, що агресивне зниження рівня холестерину супроводжується регресом атеросклерозу та зменшенням частоти серцево-судинних подій. Сучасні рекомендації міжнародних кардіологічних товариств пропонують цільові рівні ЛПНЩ <1,4 ммоль/л для пацієнтів дуже високого ризику. Використання високодозових статинів, а також комбінованої терапії з езетимібом або інгібіторами PCSK9 дозволяє досягти більш вираженого ліпідознижувального ефекту. Таким чином, інтенсивна гіполіпідемічна терапія є важливою складовою вторинної профілактики серцево-судинних ускладнень у пацієнтів зі стабільною ІХС.
